CHU de Guyane : les hôpitaux accélèrent

Le quatrième séminaire du groupement hospitalier de territoire s’est tenu vendredi après-midi et samedi matin, à Cayenne. Centré sur la préparation du CHU de Guyane, il a marqué une accélération dans la dynamique du projet. A l’issue des travaux, les participants ont acté le principe de créer une CME préfiguratrice, laquelle fera aboutir le projet médical d’ici à la mi-2023.

Après des échanges de courriers, ces dernières semaines, sur la meilleure gouvernance du futur CHRU de Guyane, un séminaire en présentiel était nécessaire, pour permettre aux responsables médicaux, soignants et administratifs d’échanger et de construire ensemble. Au préalable, une quarantaine d’entre eux avaient répondu à un questionnaire, dont les résultats présentés en séance témoignaient d’un sentiment partagé : l’urgence et la volonté de donner « un vrai coup d’accélérateur » au projet de CHRU. Avec des priorités communes aux répondants, de tous les hopitaux : renforcer l’attractivité, élaborer un projet médical, développer des équipes territoriales par filière de soins et régler la question de la gouvernance.

Accélérer le projet de CHU ? Ils ont été servis ! Car c’est un séminaire intensif qui les a réunis vendredi après-midi et samedi matin à Cayenne. Au menu des discussions : les réalisations de l’année, des débats et ateliers sur la formation, la recherche, la gouvernance, le projet médical.

C’est d’ailleurs sur les questions médicales - au cœur du projet de CHU - que le séminaire a particulièrement avancé. Les participants ont décidé que dès janvier, une commission médicale d’établissement préfiguratrice du futur CHRU succédera à la commission médicale du groupement hospitalier de territoire (GHT). Sa composition sera arrêtée par les trois hôpitaux, et permettra à la fois de refléter l’ambition hospitalo-universitaire et d’inclure les sites de façon équilibrée.

Les participants ont aussi convenu que des choix de développement clairs devront être faits et ont défini les critères de priorité : il faut développer les filières pour lesquelles la Guyane dispose d’une « masse critique suffisante » de patients, et celles pour lesquelles l’absence d’offre crée une perte de chance au regard de l’urgence des prises en charge caractéristiques de la discipline.

Par ailleurs, pour chaque filière de soins (urgences, gynécologie, psychiatrie…) un pilote sera désigné. les pilotes seront chargés de rédiger le projet médical leur filière : soins, formation, recherche. Certains projets de filière sont d’ailleurs déjà quasi aboutis, d’autres auront besoin de davantage de soutien. Au global, l’objectif collectif a été défini : que le projet médical du futur CHRU soit arrêté à la fin du premier semestre 2023.

Outre ces débats et ateliers sur les sujets médicaux, les participants ont aussi pu approfondir ensemble les questions de gouvernance permettant d’aboutir au CHRU. A ce stade, trois scénarios sont étudiés :

La fusion juridique des trois établissements en même temps ;
L’intégration progressive : l’hôpital de Cayenne devient CHR en premier lieu, puis fusionne avec le CHK puis le CHOG
La création d’une « enveloppe commune », c’est-à-dire la constitution d’un groupement de coopération sanitaire Établissement de Santé (GCS ES) c’est-à-dire une structure juridique nouvelle, dont les trois hôpitaux seraient « actionnaires ».

Les participants ont estimé que ces questions nécessitaient des approfondissements et qu’il fallait notamment analyser les conditions de réussite de chaque scénario. Ils se sont donnés jusqu’à fin mars pour avancer sur ces questions et choisir un scénario collectif.

Quel que soit le scénario qui sera retenu, la finalisation du projet médical est apparue comme un préalable indispensable. Les prochains mois s’annoncent donc décisifs et le travail acharné. Ils donneront une idée claire de ce à quoi ressemblera le futur CHRU de Guyane.

The fourth seminar of the territorial hospital group was held on Friday afternoon and Saturday morning in Cayenne. Focused on the preparation of the University Hospital of Guyana, it marked an acceleration in the dynamics of the project. At the end of the work, the participants agreed on the principle of creating a prefiguring CME, which will bring the medical project to a successful conclusion by mid-2023.

After exchanges of letters, in recent weeks, on the best governance of the future CHRU of Guyana, a face-to-face seminar was necessary, to allow medical, nursing and administrative managers to discuss and build together. Beforehand, about forty of them had answered a questionnaire, the results of which, presented during the session, testified to a shared feeling: the urgency and the desire to give “a real boost” to the CHRU project. With priorities common to the respondents, from all the hospitals: strengthening attractiveness, developing a medical project, developing territorial teams by care sector and settling the question of governance.

Accelerating the CHU project? They have been served! Because it was an intensive seminar that brought them together on Friday afternoon and Saturday morning in Cayenne. On the discussion menu: the achievements of the year, debates and workshops on training, research, governance, the medical project.

It is also on medical issues - at the heart of the CHU project - that the seminar has made particular progress. The participants decided that from January, a medical commission of establishment prefiguring the future CHRU will succeed the medical commission of the hospital group of territory (GHT). Its composition will be decided by the three hospitals, and will allow both to reflect the hospital-university ambition and to include the sites in a balanced way.

The participants also agreed that clear development choices will have to be made and defined the priority criteria: it is necessary to develop the sectors for which French Guiana has a "sufficient critical mass" of patients, and those for which the absence of an offer creates a loss of opportunity with regard to the urgency of the treatment characteristic of the discipline.

Furthermore, for each line of care (emergency, gynecology, psychiatry, etc.), a pilot will be designated. the pilots will be responsible for drafting the medical project for their sector: care, training, research. Some sector projects are already almost complete, others will need more support. Overall, the collective objective has been defined: that the medical project of the future CHRU be stopped at the end of the first half of 2023.

In addition to these debates and workshops on medical subjects, the participants were also able to deepen together the questions of governance leading to the CHRU. At this stage, three scenarios are studied:

The legal merger of the three establishments at the same time;
Gradual integration: Cayenne hospital first becomes CHR, then merges with CHK and then CHOG
The creation of a "common envelope", i.e. the creation of a Health Establishment health cooperation group (GCS ES), i.e. a new legal structure, whose three hospitals would be “shareholders”.

The participants felt that these questions required further investigation and that it was necessary in particular to analyze the conditions for the success of each scenario. They have given themselves until the end of March to move forward on these issues and choose a collective scenario.

Whatever the scenario that will be chosen, the finalization of the medical project appeared to be an essential prerequisite. The next few months will therefore be decisive and hard work. They will give a clear idea of what the future CHRU of Guyana will look like.
