Que sait-on sur le Paxlovid, qui arrive en Guyane ?

Les premières boîtes de l’antiviral seront livrées dans les prochains jours dans les centres hospitaliers et dans les pharmacies d’officine. Il peut être prescrit aux adultes ne nécessitant pas d’oxygénothérapie et étant à risque élevé d’évolution vers une forme grave de Covid-19.

L’administration par voie orale de Paxlovid doit être initiée dans les 5 jours suivants l’apparition des symptômes. Les données disponibles pour évaluer l’efficacité de ce traitement ont démontré une réduction du risque de progression vers une forme grave de Covid-19 (hospitalisation ou décès) d’environ 85,2 % après son administration. Les prescripteurs sont invités à une grande attention aux contre-indications et aux interactions avec l’un de ses composants, le ritonavir.

Le Paxlovid, qu’est-ce que c’est ?

Le Paxlovid est le premier antiviral à bénéficier d’une autorisation d’accès précoce dans le cadre du traitement du Covid-19. Conçu par le laboratoire Pfizer, il associe le nirmatrelvir (PF-07321332) et le ritonavir.

Le premier empêche la protéine du virus de se répliquer. Le second est un inhibiteur de protéase, utilisé pour le traitement des infections par le VIH.

Où sera-t-il disponible ?

Les commandes ont été passées par l’État, via Santé publique France. De premières boîtes seront livrées cette fin de semaine ou en début de semaine prochaine, dans les trois hôpitaux publics de Guyane, ainsi que chez SPG. Le grossiste-répartiteur pourra livrer les pharmacies en fonction de leurs besoins.

A quels patients s’adressent-ils ?

Pour être éligible à l’accès précoce, un message DGS-Urgent précise que le patient doit remplir l’ensemble des critères suivants :

  • Être majeur ;
  • Avoir été testé positif au Sars-CoV-2 par RT-PCR ou test antigénique ;
  • Avoir de premiers symptômes attribuables au Covid-19 depuis moins de cinq jours ;
  • Ne pas nécessiter d’oxygénothérapie du fait du Covid-19 ;
  • Être à risque élevé d'évolution vers une forme sévère de Covid-19
  • Ne pas avoir de contre-indication au traitement ;
  • Avoir bénéficié d’une évaluation des interactions médicamenteuses par le prescripteur ;
  • Avoir été clairement informé des modalités de mise en place de ce traitement

Qui peut le prescrire ?

Tout médecin à condition qu’il se conforme aux obligations relevant de l’accès précoce. Il est disponible en pharmacie de ville et en pharmacie à usage intérieur. Le prescripteur vérifie l’éligibilité du patient au traitement et effectue la demande d’accès précoce qui permettra de générer la prescription avec un code barre spécifique à la demande effectuée. Les données sont recueillies via une plateforme Web dédiée www.ap-paxlovid.com. Lorsque le prescripteur souhaite instaurer le traitement, il est invité à s’authentifier sur la plateforme via Pro Santé Connect afin d’avoir accès à l’espace sécurisé permettant de saisir les données. Il est recommandé que le prescripteur, ou le cas échéant le pharmacien, oriente le patient vers le circuit de dispensation le plus optimal, en prenant en compte le délai de 12h à 24h nécessaire pour la réception du traitement en pharmacie (hors dimanche et jours fériés). Le pharmacien passe sa commande après remise de l’ordonnance. L’antiviral se conserve à température ambiante, sans dépasser 25° C.

Le Paxlovid peut être prescrit aux patients admis aux urgences.

Quelle est sa posologie ?

La posologie recommandée pour Paxlovid est de 300 mg de PF-07321332 (2 comprimés dosés à 150 mg) et de 100 mg de ritonavir (1 comprimé dosé à 100 mg), qui doivent être pris ensemble (3 comprimés en tout, par voie orale, 2 fois par jour pendant 5 jours). Paxlovid peut être pris avec ou sans nourriture. Les comprimés doivent être avalés entiers et ne doivent pas être mâchés, cassés ou écrasés.

Quelle est son efficacité ?

Pfizer a publié plusieurs résultats d’essais cliniques. Le Paxlovid se révèle efficace à plus de 85 % à empêcher les formes graves ou les décès. Dans l’un d’eux, il y a eu 5 décès sur 697 (0,7 %) patients traités au Paxlovid contre 44 sur 682 (6,4 %) dans le groupe placebo. Il semble également efficace contre le variant omicron selon les essais en laboratoire.

Paxlovid, what is it?

What patients are they aimed at?

To be eligible for early access, a DGS-Urgent message specifies that the patient must meet all of the following criteria:

  • To be of age ;
  • Have tested positive for Sars-CoV-2 by RT-PCR or antigen test;
  • Have first symptoms attributable to Covid-19 for less than five days;
  • Do not require oxygen therapy due to Covid-19;
  • Being at high risk of progressing to a severe form of Covid-19
  • Do not have any contraindication to the treatment;
  • Have benefited from an assessment of drug interactions by the prescriber;
  • Have been clearly informed of the procedures for setting up this processing

Who can prescribe it?

Any doctor provided that he complies with the obligations relating to early access. It is available in town pharmacies and in pharmacies for indoor use. The prescriber verifies the patient's eligibility for the treatment and makes the early access request which will generate the prescription with a bar code specific to the request made. The data is collected via a dedicated web platform www.ap-paxlovid.com. When the prescriber wishes to initiate the treatment, he is invited to authenticate himself on the platform via Pro Santé Connect in order to have access to the secure space for entering the data. It is recommended that the prescriber, or if necessary the pharmacist, direct the patient towards the most optimal dispensing circuit, taking into account the period of 12h to 24h necessary for reception of the treatment in the pharmacy (excluding Sundays and public holidays). ). The pharmacist places his order after delivery of the prescription. The antiviral can be stored at room temperature, without exceeding 25°C.

Paxlovid can be prescribed to patients admitted to the emergency room.

What is its dosage?

The recommended dosage for Paxlovid is 300 mg of PF-07321332 (2 x 150 mg tablets) and 100 mg of ritonavir (1 x 100 mg tablet), which should be taken together (3 tablets in total, orally , twice a day for 5 days). Paxlovid can be taken with or without food. The tablets should be swallowed whole and should not be chewed, broken or crushed.

How effective is it?

Pfizer has released several clinical trial results. Paxlovid is more than 85% effective in preventing severe forms or death. In one of them, there were 5 deaths out of 697 (0.7%) patients treated with Paxlovid compared with 44 out of 682 (6.4%) in the placebo group. It also appears to be effective against the omicron variant according to laboratory tests.
