3 doses vaccinales avec 4 semaines d’intervalle pour les patients immunodéprimés

Une patiente guyanaise sous immunosuppresseurs a interrogé l’Agence Régionale de Santé, voici quelques jours, au sujet du délai avant la troisième dose de vaccin. Ses discussions avec son médecin ont laissé entrevoir des doutes à ce sujet. La recommandation est de réaliser trois injections avec quatre semaines d’intervalle entre chaque : la deuxième dose est donc administrée quatre semaines après la première et la troisième quatre semaines après la deuxième. En Guyane, plus d'une dizaine de patients immunodéprimés ont déjà reçu leur troisième dose.

Qui sont les patients concernés ?

Peuvent bénéficier d’une troisième dose de vaccin, les personnes :

Ayant reçu une transplantation d’organe ou de cellules souches hématopoïétiques ;
Sous chimiothérapie lymphopéniante ;
Traitées par des médicaments immunosuppresseurs forts, comme les antimétabolites (cellcept, myfortic, mycophénolate mofétil, imurel, azathioprine) et les AntiCD20 (rituximab : Mabthera, Rixathon, Truxima) ;
Dialysées chroniques après avis de leur médecin traitant qui décidera de la nécessité des examens adaptés ;
Atteintes de leucémie lymphoïde chronique ou de certains types de lymphomes traités par anti-CD20 ;
Au cas par cas, les personnes sous immunosuppresseurs ne relevant pas des catégories susmentionnées ou porteuses d’un déficit immunitaire primitif.

Qu’en est-il des patients immunodéprimés ayant contracté le Covid-19 ?

S’agissant des personnes immunodéprimées, avoir contracté le Covid-19, que ce soit avant ou après la première injection, ne change rien au schéma vaccinal : elles doivent suivre le schéma vaccinal habituel, c’est-à-dire recevoir leurs trois doses. La première dose peut être administrée à partir de deux mois après l’infection. La deuxième dose suit quatre semaines après la première, et la troisième quatre semaines après la deuxième.

Une attention particulière pour leur entourage

La Haute Autorité de santé (HAS) et le Conseil d’orientation de la stratégie vaccinale soulignent l’intérêt de la stratégie de cocooning pour protéger les personnes immunodéprimées : il s’agit de renforcer la protection de leur entourage pour réduire les risques de le contaminer. Il y a donc un intérêt majeur à encourager la vaccination des proches du patient. Sont concernés :

Les personnes résidant sous le même toit et âgées de plus de 12 ans ;
Les personnes à son contact, de manière régulière et fréquente, à titre professionnel ou non, pour accomplir à son domicile des activités de la vie quotidienne.

Who are the affected patients?

The following may benefit from a third dose of the vaccine:

Having received an organ or hematopoietic stem cell transplant;
Under lymphopenic chemotherapy;
Treated with strong immunosuppressive drugs, such as antimetabolites (cellcept, myfortic, mycophenolate mofetil, imurel, azathioprine) and AntiCD20 (rituximab: Mabthera, Rixathon, Truxima);
Chronic dialysis after advice from their attending physician who will decide on the need for appropriate examinations;
Chronic lymphoid leukemia or certain types of lymphoma treated with anti-CD20;
On a case-by-case basis, people on immunosuppressants not falling within the above categories or with primary immune deficiency.

What about immunocompromised patients who contracted Covid-19?

As regards immunocompromised people, having contracted Covid-19, whether before or after the first injection, does not change the vaccination schedule: they must follow the usual vaccination schedule, that is to say receive their three doses . The first dose can be given from two months after infection. The second dose follows four weeks after the first, and the third four weeks after the second.

Special attention for those around them

The High Authority for Health (HAS) and the Vaccine Strategy Orientation Council underline the value of the cocooning strategy to protect immunocompromised people: it involves strengthening the protection of those around them to reduce the risk of it. contaminate. There is therefore a major interest in encouraging the vaccination of relatives of the patient. Are concerned :

People residing under the same roof and over 12 years old;
The people in contact with him, on a regular and frequent basis, professionally or not, to carry out activities of daily life at his home.
